Deep Plane Facelift vs. Traditional Facelift: What’s the Difference?

What Does a Facelift Treat?

A facelift targets multiple signs of aging in the mid-to-lower face and neck. These may include:

  • Sagging skin along the jawline, often called jowls 
  • Deep nasolabial folds running from the sides of the nose to the corners of the mouth 
  • Loose skin and excess fat under the chin and on the neck 
  • Loss of definition along the cheeks and lower face 

While a facelift focuses on repositioning and tightening facial tissues, it does not typically address aging around the eyes or forehead.

Deep Plane Facelift vs. Traditional Facelift: What’s the Difference?

The main difference between these two facelift techniques lies in how deeply the underlying tissues are repositioned.

A traditional facelift generally lifts and tightens the skin and a deeper layer called the SMAS (superficial musculoaponeurotic system). By elevating the SMAS and removing excess skin, this method reduces sagging and creates a smoother contour.

In contrast, a deep plane facelift goes beneath the SMAS layer, freeing and repositioning the deeper facial structures and ligaments that have shifted over time. This technique lifts not just the skin and superficial tissues but also the underlying muscles and fat pads. 

The result is a more comprehensive, natural-looking rejuvenation, especially around the cheeks and nasolabial folds.

Because a deep plane facelift works on deeper layers without placing tension on the skin itself, it often avoids the overly tight or “pulled” look sometimes associated with older facelift techniques. The result is a softer, more refreshed appearance that better reflects how you feel inside.

How Long is Recovery After a Deep Plane Facelift?

Recovery after a deep plane facelift can vary from person to person, depending on individual healing and whether other procedures are performed at the same time.

Most people can expect:

  • First week: Bruising and swelling are most noticeable, and rest is important. 
  • Weeks 2–3: Swelling gradually improves, and many return to non-strenuous work or social activities. 
  • Weeks 4–6: Bruising typically resolves, and swelling continues to subside. 
  • After six weeks: Most of the swelling has settled, revealing a more refined result.

Can I Have Other Procedures With a Facelift?

  • Eyelid surgery (blepharoplasty): Improves sagging or puffy eyelids. 
  • Brow lift: Elevates the eyebrows and smooths forehead lines. This can also include a temple lift or endoscopic (minimal scar) brow lift.  
  • Fat transfer: Restores lost facial volume in areas like the cheeks or temples. 
  • Skin resurfacing: Treatments like laser resurfacing can enhance skin tone and texture. 

Combining procedures can reduce overall downtime compared to having them separately and help create a balanced, natural-looking result.

How Long Does a Deep Plane Facelift Last?

A deep plane facelift is often praised for its longevity. The surgical result will turn back the clock and may create a more youthful appearance and make you look approximately 10 years younger. However, you will continue to age. 

The surgical result is permanent in the sense that you will always look younger than if you had not proceeded with surgery. However, you will continue to age and may benefit from another surgical lift several years later.

Lifestyle choices, genetics, skin quality, and sun exposure can all affect how long results last. Maintaining a healthy skincare routine and protecting your skin from the sun can help preserve your rejuvenated look for many years.

Am I a Good Candidate for a Deep Plane Facelift?

Ideal candidates for a deep plane facelift generally include people who:

  • Have moderate to significant sagging in the cheeks, jawline, and neck

  • Want natural-looking results rather than a tightened appearance 
  • Have realistic expectations about what surgery can achieve
