Upper vs. Lower Blepharoplasty: What’s the Difference?

Your eyes often reveal the earliest and most noticeable signs of aging. Loose skin, puffiness, and under-eye bags can make you look tired or older than you feel. Blepharoplasty, or eyelid surgery, offers an effective way to restore a refreshed and youthful appearance. But what exactly is the difference between upper and lower blepharoplasty? Understanding each procedure can help you decide which is right for your unique concerns.

What is Blepharoplasty or Eyelid Surgery?

Blepharoplasty is a surgical procedure that rejuvenates the appearance of the eyelids. It can be performed on the upper eyelids, lower eyelids, or both, depending on each person’s needs. By removing or repositioning excess skin and fat, eyelid surgery smooths wrinkles, reduces puffiness, and creates a more open, rested look around the eyes.

Eyelid surgery is often chosen by people who feel their eyes look heavy, tired, or aged despite feeling energetic. In some cases, excess skin on the upper eyelids can even interfere with vision, making blepharoplasty both a functional and aesthetic improvement.

Upper vs. Lower Blepharoplasty: What’s the Difference?

With age, the skin in the upper eyelid area can become loose and saggy, sometimes creating folds that rest on the eyelashes. Upper blepharoplasty removes this excess skin and may adjust underlying fat to reduce puffiness. The result is a smoother, firmer eyelid contour that looks brighter and more youthful.

Lower blepharoplasty targets the area beneath the eyes. Many people develop under-eye bags, puffiness, or hollows that create shadows and make them look tired. Lower blepharoplasty can remove or reposition fat deposits and tighten loose skin, improving under-eye smoothness and reducing dark circles.

While both procedures improve the appearance of the eyes, they address different concerns. Some people choose to have both upper and lower blepharoplasty for a complete eyelid rejuvenation, while others focus only on the area that bothers them most.

How Long is Recovery After Upper or Lower Blepharoplasty?

Recovery after blepharoplasty is usually straightforward. Most people experience bruising, swelling, and mild discomfort in the first few days. Cold compresses and keeping the head elevated help reduce swelling. Stitches are typically removed within five to seven days.

Most people feel comfortable returning to normal daily activities within seven to ten days, though swelling and minor bruising can last a bit longer. Strenuous exercise and heavy lifting should be avoided for about two weeks. Makeup can usually be worn once healing skin feels comfortable and your surgeon gives approval.

Upper and lower blepharoplasty recovery timelines are similar, although some people may notice slightly longer swelling in the lower eyelids.

How Long Do Eyelid Surgery Results Last?

Eyelid surgery delivers long-lasting improvements. Results from upper blepharoplasty can often last ten years or more. Because the skin continues to age naturally, some looseness may return over time, but it is usually less pronounced.

Lower blepharoplasty results are often even longer-lasting. Once under-eye bags or fat pockets are removed or repositioned, they rarely return in the same way. Good skincare habits, sun protection, and a healthy lifestyle can help extend and protect your results.

Am I a Good Candidate for Blepharoplasty?

Blepharoplasty may be a good choice if you:

  • Have sagging skin on your upper eyelids that creates a heavy or tired look
  • Experience puffiness or under-eye bags that don’t improve with rest or skincare
  • Are in good general health and do not smoke
  • Have realistic expectations about the results
  • Want to look more refreshed and alert without changing your natural expression

During your consultation at Specialty Aesthetic Surgery, your surgeon will examine your eyelids, discuss your concerns, and recommend the best approach for you. Some people may benefit from combining blepharoplasty with other procedures, such as a brow lift or facelift, for more comprehensive rejuvenation.

Aging around the eyes is common, but it doesn’t have to define how you look or feel. With upper or lower blepharoplasty, you can achieve a naturally refreshed appearance that helps your eyes reflect your inner vitality.
